I just noticed, look at the other side shot, the whole head is melted and smushed in!

Stored somewhere where it began to melt - melting point of the plastic anyway. Can fix the crack with plasti-welding. If very careful you might be able to re-form the nozzle by heating it close to pliability and then bracing it to the proper shape while it cools down, or just leave as-is.
