It has a large heavily sprung disc inside that under vacuum suction rapidly opens and closes. Same principle as an organ's Diaphone pedal piperank which employs air pressure for the same beater effect. With graduated open resonators a musical tone of great presence is produced. Placed at the end of the hose, it is the hose itself which vibrates (and does indeed emit a tone of low frequency). One wraps the hose around the affected limb for relaxing vibrational massage. I'd let the FQ instruction on the box explain the rest but I can't find that scan...
