It's a CenTec CT12DX power nozzle with out headlight.

This link is for the hybrid bare floor brush built into power head nozzle as found on blue Kenmore Intuition.
I do not recommend this power nozzle. The bare floor brush has few bristles and rear wheels break off. Connection to power nozzle is flimsy and has broken for many customers. Power nozzle itself is same motor and agitator as other CenTec heads but it's a very bulky head, that is hard to maneuver and is clumsy. Does handle and clean nearly as well as other nozzles.
